A healthy community is better for everyone

SWPN’s health strategy is to improve the physical and mental health of all South Ward residents and to build the community’s first federally qualified health center. The Clinton Hill Community Health Center, which will be largely focused on early childhood development, is slated to open its doors in 2022.

Currently, we offer two programs to address health issues among children including a kids fit program and in-school asthma treatment support.

COVID Vaccine Information

The federal government is providing the vaccine free of charge to all people living in the United States, regardless of their immigration or health insurance status. Download the flyer for more details.
