SWPN’s Promise Navigation Program, using a two-generation approach, links families to a pipeline of twenty-six (26) partner organizations addressing a wide range of supports, services and resources in the key life domains of family, education, health and wellness behavioral health. Promise Navigation is a whole family system of care that supports families to knock down barriers and guide the family’s journey towards thriving, including education success, asset building, workforce development and achieving key life milestones.
Promise Navigation uses a team approach to provide domain specialists and community health workers (CHWs) who work alongside families from cradle-to-career to develop, navigate and connect individuals to resources through two concurrent service lines—the Stability Service for delivery of highly intensive, customized family support and direct services and the Milestone Service for coaching and support in which
CHWs proactively “walk with” family members to promote success at key Milestones.
Promise Navigation also provides emergency assistance in the areas of rent, security deposits, utilities, food, and supplies. Individuals may reach out via the SWPN Text Support Hotline to join the Promise Navigation Program and/or to seek emergency assistance.
